Our Complex Care service provides high-level, long-term support for individuals with significant and ongoing healthcare needs. These needs may arise from chronic conditions, disabilities, neurological illnesses, or recovery following hospital treatment. Our Complex Care teams are specially trained and overseen by clinical leads to ensure every aspect of care is delivered safely, effectively, and in line with best practice. We work in close partnership with district nurses, hospital discharge teams, physiotherapists, occupational therapists, specialist consultants, and community healthcare professionals.
This joined-up approach ensures your care plan is clinically robust, regularly reviewed, and aligned with your long-term health goals. Our aim is to support you to live safely at home, reduce avoidable hospital admissions, and maintain the highest possible quality of life.
